WELCOME TO YOUR NEW GLP-1 BENEFIT PROVIDED BY YOUR EMPLOYER

Beginning August 17, 2026, your employer will provide each eligible employee who meets the clinical criteria with up to $300 per month to use toward the cost of Zepbound Pen or Wegovy Pen. These funds can be used via a pre-paid Visa® debit card known as TheRxCard at St. Luke's pharmacies as well as most major retail and independent pharmacies during the checkout process.

Members with Active Current Authorization:
If you have a current approved prior authorization with a prescription that has been filled since April 1, 2026 with Capital Rx, your authorization will transfer to TheRxCard, and your provider does not need to submit your prescription for a new authorization until your current authorization expires. When it expires, your provider will need to submit for a new authorization to confirm you still qualify for the program. Your prepaid Visa debit card from TheRxCard will arrive in the mail within 15 business days.

Members without Active Current Authorization:
To begin a weight-loss GLP-1 medication, you must first meet with your provider to determine if you meet the eligibility criteria for coverage, including the initial authorization requirements for patients 18 and older:

• Meet one of the following BMI requirements
  • BMI > 35 with no comorbidities
  • BMI > 30 with at least one comorbidity (Hyperlipidemia, Hypertension, Obstructive Sleep Apnea, Cardiovascular disease, Type 2 diabetes, Nonalcoholic fatty liver disease/MASH, Osteoarthritis of knee/hip/back, Moderate/Severe Asthma or Reactive airway disease)
• Three (3) health coaching sessions within the initial 6-month authorization period.
  • Failure to complete sessions within 6 months will require a full restart
  • No call, no show incidents require restarting all sessions
• Patients between ages 12 and 18 do not need to attend health coaching sessions.
